To register for an appointment you have already scheduled, please select your injury type from the menu below.

To Make an Appointment

We make scheduling an appointment and registering for treatment easy! To make an appointment:

1. Call or email your local Partners at Jen@partnersinrehab.com to make an appointment. Include preferred dates/times in emails.
2. New patients must register either online or on paper. To register on paper and still save time, you may download, print and fill out the Registration Forms available here. The form may also be completed at the time of your appointment.

To register online, please select your injury type from the pull down menu at the top of the page. All registration forms are processed via Secured Server protections to maintain the safety of your information.

Returning patients need not register again. Please notify the office that you have been here before and simply verify your information on your first visit.

Partners offers convenient early morning and evening appointments in three local facilities. Appointments are available within 48 hours of referral.

 

Insurance

Partners accepts Medicare, HMO's, all private insurance and workers compensation.

 

Referrals

Most insurance plans require a doctor's referral, however patients in Massachusetts have the right to direct access and are free to select a Physical Therapy practice independently.

Please contact your insurance carrier to ensure any required referral is available at the time of your appointment.

Call or email your local Partners to make a convenient appointment. Include prefered dates and times.

Obtain a referral from your doctor, if required by your insurance carrier.
